Online appointment forms
Please use for Routine appointments only
As part of improving access for any future, planned care, we have an alternative to using the telephone for people who prefer online services.
The Telephone is still our preferred option, if you need help today or tomorrow, so we can assess the urgency and ensure that you get the right help. Our average call wait time is less than 2 minutes, and our team is trained to ask for information to help the decide which clinicians can help and to decide who needs help soonest.
We use the same system for online requests as our messaging system, so that you are familiar with the company we use – Accurx. If you want to see how the system works and check the help system, you can find more information on the patient resource page.
To use this new form, it can be accessed either from our Routine Appointment page or via the NHS App if you are registered. You can also request Sick (Fit) notes, or order repeat medication, and enquire about your test results. You can also access this option via the NHS App.
If you unsure and would like to see what happens with your enquiry before using the system, this video will help.
Ideas, concerns and expectations – So that you are prepared before using the system, please also watch this ideas, concerns and expectations patient video to get more advice about sending us the right level of detail. This helps us to make sure we have the right information about your query and can signpost you to either the right person or service, and to decide who needs help soonest.
Continuity of care is as important to us as it is you. As a small practice, we may not have appointments with your preferred clinician when you want, but you can also tell us on the form who you would prefer to see and we will attempt to arrange this, depending on how soon you need a reply.
The online appointment form will be open 8am to 18:30pm, and we will respond to your enquiry within 2 working days.
